Greenfield Physical Therapy is one of the only physical therapy practices in the entire country which has a male physical therapy practitioner specializing in pelvic floor conditions. Dr. Jake Greenfield is proud to specialize in men’s pelvic health.
Additionally, Dr. Greenfield has a lot of experience treating female patients, and patients of all gender identities. Some patients prefer to have a male practitioner and our practice is inclusive to all genders.

During your free consultation, we briefly discuss what health complaints you have and why you are seeking physical therapy services. We talk through whether Greenfield Physical Therapy is appropriate for you, or if you need to discuss your issues with a different healthcare provider.
At your initial assessment, we have a more in depth discussion about your medical history, and perform some movement assessments to provide a better picture of your global health. We then determine an appropriate plan of care for physical therapy treatment to help you achieve your goals.

Greenfield Physical Therapy values direct one-on-one care which is tailored to address the individual’s global health. To achieve this high level of care, we operate as an Out-Of-Network or “Cash-Based” clinic. This means Greenfield Physical Therapy does not take your insurance, you pay the clinic directly at the time of service. We do provide you the option to have a superbill. You can send this document to your insurance company, and they may reimburse you directly depending on your plan's out-of-network benefits.
If you are engaging in our Health & Wellness services, there is no option for a superbill as insurance will not reimburse these services.

Under California law, you may receive direct physical therapy treatment services, without any outside approval, for a period of up to 45 calendar days or 12 visits, whichever occurs first.
Afterwards, we need your physician/surgeon to sign off on the physical therapy plan of care to continue treatment. We require your primary provider’s information at the time of your initial assessment so we can send them this plan of care after the first visit is completed. This helps to expedite the process. California law also requires you, the patient, to meet with your provider (physician/surgeon) so they may evaluate you in person or through telehealth, to approve the physical therapy plan of care.
If you are engaging in our Health & Wellness services you do not need to meet with a physician/surgeon and they will not need to sign a plan of care.

Absolutely! We have a lot of experience treating patients through telehealth for a variety of conditions. Many patients prefer telehealth treatment because they feel more comfortable in their own space, save time by not commuting for a medical visit, and relieves the stress of finding childcare. This gives you more opportunities to get the treatment you need.

Physical Therapy is meant to address impairments, symptoms, injuries, conditions. On the other hand, Health & Wellness services are meant to prevent these issues entirely. Working with a physical therapist through Health & wellness services, can improve the years a person lives in good health, is active, independent, free from chronic disease or serious disability.
Additionally, a benefit to these services is that no physician/surgeon referral is necessary.
